Physicals on May 9

The Athletic Training and Sports Medicine Department will again be providing physicals for the BT student body on Monday May 9 from 3:30pm until 6:00pm.

Cost for physicals is $30.00.  Please make check payable to Blessed Trinity Catholic High School. For current Blessed Trinity students the cost of the physical will be charged to their FACTS account.

Students, elementary age through high school, are invited to have a physical done for the upcoming 2016 – 2017 school year. Any high school student participating in a Georgia High School Association (GHSA) sanctioned activity is required to have an annual physical. Such activities include athletic teams, cheerleading, dance, drama, band, debate, and academic team. GHSA mandates that all these participants must complete an annual physical using the GHSA forms. Additionally, all incoming freshman students are required to have a valid GHSA physical. Physicals completed on non-GHSA forms will not be accepted. The physical will be valid through the entire 2016 -2017 school year and athletic seasons. The physicals are set up into eight stations: Pre-registration, Registration/check-in, Height and weight measurements, eye exam, Blood pressure and pulse, Orthopaedic screen, Medical clearance and Check-out. Requirements at registration and to attend:

  • Completed and signed GHSA History Questionnaire by parent and student. The remaining forms will be completed and signed by the physician at the conclusion of the physical. Please be sure all original forms are turned in at check out. You may request a photo copy of forms at check out.
  • Cash or check made payable to Blessed Trinity Catholic High School for $30. FACTS account charged for current BT students.
  • Proper athletic attire such as shorts, t-shirt, females may be more comfortable wearing a sports bra under the t-shirt.

We do not make appointments. Please understand that coming early only increases your wait time. In-season BT athletes will be given priority. In order to avoid waiting in a long line, please use the following time guidelines. Arrival times: 3:30 – 3:50  – BT in-season athletes 3:50 – 4:30  – rising seniors 4:30 – 5:00  – rising juniors 5:00 – 5:30  – rising sophomores 5:30 – 6:00  – rising freshman, middle and elementary-aged students We thank you in advance for coming to Blessed Trinity Catholic High School and having your student’s physical completed by our Sports Medicine Team of professionals.
